What to have ready Have the tax-return name, mailing address and TIN for the person or entity on line 1 ready. For a sole proprietor or disregarded entity, use the owner’s name and applicable TIN. Check the official federal tax classification and exemption instructions before choosing an answer.
Name, classification and TIN should identify the same taxpayer
First confirm who belongs on line 1, then check line 3a and Part I. Select a label to locate each entry on the original form.

This detail is from the official English original. Labels identify locations and do not change the original form.
1Line 1 · Name
Use the tax-return name of the person or entity whose TIN you provide. Sole proprietors and disregarded entities must follow the owner-name rules.
2Line 3a · Tax classification
Choose the federal tax classification of the person or entity on line 1. Do not choose solely from a business name or an LLC registration.
3Part I · TIN
Enter the TIN that corresponds to line 1 under the official name-and-number rules. A format check does not verify your identity or government records.
Line 2 is for a different business, DBA or disregarded entity name, if applicable. Sole proprietors and disregarded entities must check the owner rules; an LLC name alone does not determine its tax classification.
Read the official name and classification instructionsLine 3b: check the classification, ownership and foreign-person conditions
Check line 3b only if all conditions apply: you are a partnership, trust, estate or LLC taxed as a partnership; you give this W-9 to a partnership, trust or estate in which you have an ownership interest; and you have direct or indirect foreign partners, owners or beneficiaries. Foreign status is a tax-status question, not simply a question of mailing address.
Exemption codes depend on the taxpayer, payment or account
Backup withholding exemption and FATCA reporting exemption have different rules. Enter a code only when the applicable taxpayer and payment or account conditions are met. Individuals, including sole proprietors, are generally not exempt from backup withholding. Check the official code definitions and exceptions.
Review the certification, then give the form to your requester
Give this form to the requester, not to the IRS. The PDF is unsigned and has not been submitted. Review Part II and the signing rules on pages 1 and 5. If the IRS has notified you that you are currently subject to backup withholding for unreported interest or dividends, follow the instructions for crossing out certification item 2 before any required signature. Complete any required signature and date, then follow your requester’s delivery requirements.

Waiting for a TIN?
The instructions on page 4 explain when to enter Applied For after applying, or when intending to apply soon, for a TIN. This editor does not support that special output. Use the official PDF and check the application, certification, signing and delivery instructions.
Read the official form and all instructionsNeed to circle a name, mark NEW or attach a statement?
Some joint accounts require circling the relevant name; an address change may require NEW at the top; and certain tax-treaty claims require an attached statement. These requirements do not apply to everyone. This editor does not produce those annotations or attachments. Review the applicable instructions on pages 2–5 and complete any required steps after downloading.
Read the official form and all instructionsNot sure whether W-9 applies to you?
W-9 is for U.S. persons as defined for tax purposes, including U.S. resident aliens and qualifying U.S. entities. A mailing address outside the United States does not determine tax status by itself. Foreign persons generally use the appropriate W-8 or Form 8233. This tool does not determine tax residency or eligibility; check the definitions and exceptions on page 2.
