What to have ready Have the filed return, taxpayer names and identification numbers, current address, refund amount and period, original payment method and any applicable bank details. For a joint return, gather both spouses’ details.
Was the refund issued, and how was it meant to arrive?
Start with the IRS payment status and the original return. A processing delay and a missing issued payment need different next steps.
Prepare a separate Form 3911 for each refund. This tool creates a PDF; it does not start a trace or send anything to the IRS.
Paper check
Confirm the issue or mailing date and the address used. If a check you actually received was lost, stolen or destroyed, contact the IRS about a trace.
For a check never received, general IRS guidance uses 4 weeks from mailing, or 9 weeks for a foreign address. Mail forwarding can require 6 weeks.
Use the refund-status instructions for your circumstances. Joint filers cannot initiate a trace through the automated tools; an IRS representative can help.
Direct deposit
Verify the routing and account details on the filed return, then contact the bank or financial institution to ask whether it received or returned the payment.
A trace generally requires at least 5 calendar days after the scheduled deposit date and prior contact with the financial institution.
A closed account, rejected deposit or refund-advance product can require a different route. Report the original payment information; Form 3911 is not a request to change bank accounts.
The return is still processing
Check the IRS refund status first. Do not use a missing-payment statement merely because the return has not finished processing.
You still hold an old check
A Treasury check cannot be cashed after one year. Contact the IRS for instructions on returning it; do not describe a check you possess as lost.
The bank returned the deposit
Follow the IRS notice or account-update instructions. An automatic replacement or a bank-account update may apply instead of starting another trace. For a refund advance or fees taken from the refund, contact the preparer or financial institution first.

Describe the facts without forcing them into one status
Section II says to check all boxes that apply. Select only statements that are true for this refund; the original refund method does not answer these questions for you.
- You did not receive the refundLine 8
- Use this statement for the missing refund. Check the payment status and the bank or mailing circumstances before returning the request.
- You received a check, then something happened to itLine 8
- Select every applicable reason: lost, stolen or destroyed. These reasons belong to the statement that you actually received the check.
- You received and signed the checkLine 9
- Report this separately, including when the check was later lost or stolen. The original warns that a replacement is not allowed if you endorsed it and someone else cashed it without forging your signature.
A trace establishes what happened; it does not guarantee another payment. If a check was cashed, the Bureau of the Fiscal Service may send a claim package and examine the endorsement.

Name and address changes are separate questions. When you explicitly confirm no change, the tool prints N/A in the corresponding name or address main field and leaves the small address boxes blank.

One refund can require two signatures
Use the signature arrangement for this refund. Typing a taxpayer’s name in the editor does not sign the certification.
| Refund situation | Line 10 | Line 11 |
|---|---|---|
| Individual, not a joint refund The taxpayer signs and dates the certification. | Taxpayer’s signature and date | Leave the spouse’s signature line blank if it does not apply. |
| Joint return Enter both spouses on lines 1 and 2 and obtain both signatures before returning the request. | First taxpayer’s signature and date | Other spouse’s signature and date |
| Business return The person authorized to sign the business’s refund check signs the certification. | Authorized person’s signature and date | The signing person’s exact business title, not a spouse’s name |
| A spouse has died Ask the IRS about the decedent-account requirements and whether Form 1310 or supporting documentation is needed. | The surviving spouse signs in their own applicable signature line. | Write Surviving Spouse on the deceased spouse’s signature line, whether line 10 or 11; do not create their signature. |
Current IRS procedures accept handwritten, electronic or digital signatures. This tool does not apply any of them or complete signature dates. An authorized Form 2848 representative may act within their authority; listing someone on lines 5–6 alone does not create that authority.

From prepared PDF to an IRS refund trace
Keep the filed return, payment information and any IRS correspondence together so the completed request describes the same refund.

Match the details
Review the current identity, the return as filed, tax period, original refund method and amount. Fill banking information when applicable, including for a check normally cashed or deposited at your bank.
Finish the certification
Download the two-page PDF, review the declaration and complete the required signatures and dates. If two refunds eventually arrive, the certification requires returning one.
Use the correct return channel
Use the IRS reply envelope or the fax instructions supplied to you, or confirm the current mailing address or fax on the official Form 3911 page. Individual and business destinations differ, and special foreign-address rules apply. Only Form 3911 should be sent to the listed fax numbers.
IRS procedural guidance generally allows 6 weeks after receipt for a paper-check trace or up to 120 days for direct-deposit tracing. These are handling time frames, not a promise of a replacement payment.
Save your completed copy and follow the IRS correspondence. Preparing or downloading this form does not mean that the IRS has received it. An address provided on Form 3911 can be used by the IRS to update its address record.
Special situations and tool limits
Can I put several missing refunds on one request?
No. Complete a separate Form 3911 for each refund. A held check that is over one year old also needs IRS return instructions rather than an inaccurate lost-check statement.
Read the official form and all instructionsWill entering a new bank account here redirect the refund?
No. This form traces the payment in question. Contact the financial institution about an incorrect deposit; follow any IRS notice or online-account instructions for a returned payment. A refund advance or fees deducted by a preparer require contacting the preparer or institution first.
Read the official form and all instructionsDoes a joint refund need both spouses to sign?
Yes. Both spouses must sign before the IRS can begin the trace, subject to the special decedent-account requirements. IRS procedures accept handwritten, electronic and digital signatures; this tool leaves the signature and date areas for completion after download.
Read the official form and all instructionsHow are my entries handled?
Your entries are processed in this browser page. Each form keeps its own draft during site navigation; refreshing or closing the page clears the drafts. Optional analytics, including Clarity recordings, follow your choice in Analytics settings and the Privacy Policy. Download a copy to keep your work.
Can I edit the downloaded PDF?
Yes. The exported PDF preserves all 2 original pages and editable fields. It is unsigned. Follow the recipient’s signing and delivery requirements.
Why might some text need review?
The original PDF has limited field space. If text cannot fit or its characters are unsupported by the PDF font, we will identify the field so you can revise it. We never silently remove characters or clip overflowing content.
